check your router and make sure you have the ports for punkbuster forwarded.

Update your pb at evenbalance also.

I've already updated pb, also tried uninstalling then reinstalling (pb not cod4). How do i forward ports for pb? I also have another problem, sometimes i get kicked for this:

Notice Jab Punkbuster kicked player 'Jab'(for 0 minutes)... Restriction: Service Communication Failure: PnkbstrB.exe driver failure (pnkbstrK.sys) ffa0

I thought that playing with xfire running could be the problem, but no luck. Would forwarding the port fix this to?

portforward.com for the forwarding ports issue. Failure of punkbusterb, that is an issue with the install of punkbuster i think.

You may want to check on that at EVENBALANCE.Com.

Forwarding the ports fixed the md5 tool query and loosing key packets errors, but I still get the pnkbstrb.exe driver failure. I've already tried reinstalling pb with no success. Here's my pnkbstrb log file, it's different from the guide (not just longer):
