the only really scary game i ever played was F.E.A.R but i think extraction point was the scariest of them all. i mean they made a hospital ever scarier. i didnt think it was possible.
Dead Space... Half Life 2 in parts (i.e. in the city area where you fight only the zombies, i dont remember the level name)... there are others, but my top is Dead Space. I turned off all the lights and turned up my surround, very surreal how well done the ambient noises are, along with the whole feel to the game. One of the best horror games out there.

"Ravenholm.... We don't go there anymore."
Best level in the fucking game, mostly because of Father Grigori.
S.T.A.K.E.R. - Shadow of Chernobyl, especially the underground labs. The first time I went through Lab x18, I had to quit the game for a bit cause I got too scared. And that was at like 11:00 a.m. in the morning.
I just played through Dead Space, it wan't scary. At all. I think the third person view destroyed the fear for me.
Doom 1 used to scare the crap out of me at the time back when I was probably 10 years old.
The only game that ever made me jump was FEAR. You'd have to be crazy not to find that scary. Unless of course it's daylight and you're not alone. Another game (actually it's a mod for a game) is Nightmare House: The Remake. The first Nightmare House (for HL1) didn't scare me at all, but the remake made me just about wet my pants. If you own Half-life 2, I suggest you download Nightmare House: The Remake (it's a free mod).
Dongens Dragon was one game that is one of my favorite games. But I lost that.. If you have that or any link to play it please provide.
