I think the worst for me ever was in Resident Evil 2 when the licker jumps out from behind the mirror... I almost broke my window throwing the controller out of shock.

Quake. The first one. So there I was, running through the map armed with the Double Barreled Shotgun (or was it the Super Nailgun... I'm not sure anymore) and my friend sitting next to me. I passed through one of the portals (teleporters of some kind) and appeared in a small, closed room with no apparent exit. Right in front of me on the floor there was a switch. At first, I didn't even see it, so I kept going forward without stopping and I stepped on that switch. Suddenly, the walls around me lowered and the giant moster called Shambler was standing in front of me. He instantly let out a blood-freezing scream and charged at me, killing me on the spot. Both me and my friend jumped back in our chairs. =p
A couple moments in Dead Space got me. When you first encounter the enemy things and your running as fast as you can since you don't have any weapons yet, the first time the giant worm thing grabs you and starts dragging you away, and the last bit at the very end of the game (people who finished it will know what i mean).
I don't remember what the game was. It's the one where you're a guy on a space station, and in the beginning he's with some woman, then a big monster jumps in and bites her head off, and then you get to play as the guy running down a big hall trying to escape the monster. Anyways, I played that game when I was younger, so at the time almost everything about it creeped me out, but there was this part where a weaker, foot soldier-type alien jumped down from the ceiling in front of me and I apparently reacted quite badly to that, according to my friend.

First time playing Stalker and having the controller, out of fucking no where take control of the camera.
I was high on some Benadryl or something at like 2 AM over the summer playing Alien vs. Predator 2 on the Marine campaign. That was the most terrifying experience I've ever experienced on a screen I swear to God.
Interestingly the way I solved it was simply playing Lady Gaga on repeat, because when you're listening to Lady Gaga it's hard to imagine that even an extra-terrestrial that rapes you and breaks your ribcage would be half as terrible as Lady Gaga.
dead space in the begining when you first meet the alian zombie things when your running from them, mind you that you have no way of defending yourself at this point, jump in the elivator and the doors close and you think your safe. Till the zombie starts ripping the door open and comes at you anyway....good stuff. Also pretty much the rest of that game.
ps. resident evil 2 on ps1 gets mention if not just for the fact you couldn't run and shoot and this made the dogs a bitch.

Agreed, although back when I played that game I was probably a bit more impressionable.
Resident Evil Zero - Going back into the Research Facility and seeing tons of primates, while I had only a pistol and some molotovs.
Dead Space - When they come at you when the elevator door opens.
