So to be clear;
Not badly optimised, so it looks terrible and runs even worse, but rather, what would you say is the game currently most pushing the boundries of graphical quality, but as a result is tough to run on most hardware currently?

I would say The Witcher 3 holds that crown. It's pretty when you just look at it, but when you start to dig into the amount of attention that has gone into all the details with the way particles move and behave and lightning works, it becomes clear that it's a visual masterpiece.
Most other good looking games that are hardware intensive have nowhere near that level of detail.
