In my experience, there are 2 things that lead to a relationship.
It depends on what you want to classify lust as, it could be lust for the person, lust for sexual 'fantasies', lust for a relationship etc... But no, Not all relationships start with lust, ever heard of love at first sight? It may sound stupid and not possible to some, but trust me, its possible
it can happen happend to a freind his devotion can be scary

I was friends with my gf for about 6 months before we got together.
It can start out as random lust, but it also can start out as friendship (which is MUCH harder than the former).

I lusted after my wife for 2 years before we went out...now I have been with her for over 8 years.
Well for me it has always been a lust thing at first. Maybe it’s a biological thing, or I just have a weakness for the flesh (yeah, probably that), but I’d always been in the mindset of making sure mutual sexual satisfaction was to the liking before exploring anything further towards a relationship. My wife and I knew each other for a couple of years and then one day – lust took over and we’ve been together ever since. Although, I must admit as years go on the relationship becomes more (obviously) and sex though important is no longer the driving force in the relationship; especially for men (yes, I know blasphemy!), but you younger bucks will figure that out in a few years, that is, assuming you’ve found someone to spend your life with.
