Sure, rape is unexpected, but doesn't the fact that you know you'll be hurt add to the torture? Imagine that someone suddenly shoots you in your kneecap. It will hurt like hell. Now imagine that same person warning you he will shoot you in the kneecap first and there's nothing you can do to stop him (you're tied to a chair, for instance).
It depends on the individual.
For some the shock numbs the pain, for others shock becomes too much and they panic.
But, largely, most people would prefer to see it coming.
