Doom3 World - Level Editing, Scripting, Models, Animation, Mods and Maps is a great place to start, you can get most of the tools ( MD5 import/export plugins ) and info on how to use them. Don't let the name fool you ( Doom3world) , as Q4 works on the D3 engine, and most of the information is just as relavant to Q4. They also have a Q4 section now, but most of the tools and even a few walkthroughs on custom model creations are found under Doom3. You will need some sort of photo editing software that you provide ( I use PhotoShop) And some sort of 3d modeling software ( I use both 3D Studio Max and Maya) but there are only so many variations, and compatability of the import/export plugins that you now have a some options. D3 & Q4 have a built in editor but I have never used it, so I don't know what you can do in it.GTK RADIANT is a cross platform map and level editor which is quite nice, but I'VE never done a map , ever . For textures , I use a DDS plugin from Nvidia ( you can download it from there main site) and it works awsome in photoshop. I used Dertons 3d max plugin for some of the modeling I have done but I think the maya plugin I used in maya version 6.0 was the easiest. There are many options available to you and Doom 3 world has all the answers you are looking for, or someone there can find one for you. You can email me at bladerunnercutter@hotmail.com . I'm a novice to alot of these things( only a couple years experience ) , but if I can, I would be glad to help.

A Good question, that I do not have the answer to. The best thing to do would be to make a post on DOOM3 World , to see if an import/export is available for milkshape, or to look around on their site. Also Go to idDevNet And click on Doom3 , then click on downloads on the top left column. You will see links to most of the import/ export for MD5 and more. If you goto the Q4dev you will find some good info on texturing and more.

Edit: Bingo, !!! I found a milkshape plugin for you , download at chUmbaLum sOft Of course I don't know how well , or if it works on Q4 ,But it is the only one I have found so far.

Edit: Swweeeeeettt !!!!
