They're case-sensitive. Try changing the names of both the mesh nodes and the mesh groups to lower case.

When you setup the hierarchy, the mesh nodes are those nodes that define the respective mesh parts (i.e., parts of the model). These following the following format: m_meshnodename#1 m_meshnodename#2 m_meshnodename#3 m_meshnodename#N - where meshnodename is the actual name of the mesh node.

If you are using Milkshape, these names will correspond to the names found under the Groups tab and you will need a mesh node for each group you find there. As you are relatively new to this process, I would recommend that you setup your hierarchy in stages…exporting and testing along the way. Keep in mind that when setting up the hierarchy, where siblings are connected DOES mater, so pay close attention to that. In the case of mesh nodes make sure each mesh node is individually connected to the h_Scene Root.

insert_name_here;4807276....I opened up a stock .SOD (of the Enterprise) and there's a "h_root" as well as an "h_scene root" is this my problem?..

......(you do have them right)but yeah, it looks like the nodes you set up don't match between what you have in your groups tab and your joints tab. Whenever I get this on mine, most of the time it's typing error. Remember that the names of your mesh groups in your groups tab has to be exactly the same with the ones in your joints tab with the exception of having this(m_)added to them of course.
