For one thing, the JKA ports listed on that website are not correct and are infact reapeats of the JK2 port list. :uhoh:

For JKA , open UDP ports 29060 through 29062 and 29070 through 29081.

For the IP, use the IP that is assigned to your comp. At least that is the way it works for me.

Use the ports I listed. I think you could get rid of the old ports you put in unless you intend to host a JK2 server. Also I don't really know about the pop up thing. I just clicked "never register" and clicked "ok" or whatever I didn't se mine again. Perhaps register it? lol.

If you are using JA+ I believe that yes, you might need to open 21000. Also, sorry that I can't be of much help about the registration thing at the moment. My JKA didn't seem to have that problem :S

Do you mean 1.0 of jka ? If so, that is the older version that very few people still play.
