Well, in the False Gods novel there's a sword designed as a Primarch Killer, could be his Talon was designed as an Emperor killer? Both survived until some other thing affected them(Horus' case being chopped in half by the emperor, the emperors case a life-support failure?)

Anyway, how can we tell that he hasn't healed while in there? Only the Custodes and those they consider truly worthy can enter the palace itself, and i know of no man that has been allowed to see the Emperor himself since about M32...(Except the Techpriests, and they're not saying a word...) Or, it could be that the Throne itself operates partially like the Stasis Field that Roboute Guillimans body is in, just freezing the person in their final moments...

Yet to see why they didn't find their Chief Apothecary and try to save Guilliman(As in, just repair what they can) if they've still got him technically alive on Macragge... Might be that it was just presumed fatal.

I've seen company captains survive a direct hit from a Vindicator, and a land speeder bring down a Chaos Banelord titan, so why can't he survive a Single stab wound? Horus survived a direct attack from the Primarch killer blade...

So ends the rant about some annoying parts of the fluff that i don't know even exist in the next edition.

I'm pretty sure the return of the emperor would mean a lot of very dead Radical Inquisitors, and more than a few Psykers, as for the Sisters of Battle and the Ecclesiarchy(not sure about spelling)...

And the Adeptus Ministorum, possibly the Mechanicus too(As they also have turned to... Religion.), just hope he doesn't know of the actions of the High Lords of terra over the past five thousand years...

Basically, for a large part of the imperium, it'll be bad news. Not sure how annoyed he'd be at the lack of his Legions, but he'd definitely want to lead another crusade(Too many Xenos and Daemonic things on imperial worlds...) Hopefully his return would decrease the rioting, having someone going round with the full force of the Adeptus Custodes, not forgetting his power to begin with...

Wouldn't be a very good time for Abbaddon the Despoiler either, a massive increase in loyalty and reckless heroics from his enemies, the sisters of silence stopping his Daemon legions from using their powers, and then the basic fact(Or propaganda at least) that even the Grey Knights aren't half as powerful as the Custodes.

And what happens if he dies? will he get reincarnated or will the Imperium be effectively screwed?

If you still believe that ancient Star-Child theory, he supposedly will be reincarnated. If you choose to ignore something that hasn't been mentioned in the actual fluff for several years, the imperium is screwed.

As the astronomican is decreasing in power (Parts of Ultima Segmentum, including Maccrage cut off completely or at least only for short times able to travel safely) and the tech-priests have found faults in the Throne they cannot repair, it's possible we won't have to wait long to find out.
